LAKE HAVASU CITY, AZ (AZFamily) — Donna Howard is breathing a little easier these days after being mistakenly declared dead and having her vital accounts shut down.

“It feels good, but it’s still just a bit stressful,” she said.

In a previous On Your Side report, Howard told us she was informed through her prescription provider that she was deceased.

The situation snowballed from there, with other notifications telling her she had been declared dead.

Even Medicare, Howard’s bank and her railroad retirement account had listed her as no longer living. “I didn’t have health insurance for months,” she said. “And they declared me deceased without a death certificate, so my pay stopped.”

Hospital mix-up triggers nightmare

Howard’s bizarre ordeal started at Henderson Hospital just outside Las Vegas.

An accident victim was brought there, and somehow, Howard’s insurance was billed for the mystery woman’s treatment. When that woman passed away, it appears the death was reported to Medicare under Howard’s name.

Medicare then shared the information with others, and that’s when Howard’s life spun out of control.

“It’s been bad,” Howard said, “It’s been bad.”

Howard shared her story with On Your Side, and we contacted Social Security, Medicare, Howard’s health insurance, her prescription coverage, her retirement plan — anyone who thought Howard was dead.

After explaining the situation and providing documentation, Howard was back among the living and all of her accounts were reinstated.

Howard doesn’t believe this would have worked out without On Your Side’s intervention.

“The very next day that this aired, I got calls from two of my insurance companies,” she said.

Advocating for change

Now that things are getting back to normal, Howard says she wants to be an advocate for others facing this situation. She created a Facebook group called “Erroneously Declared Deceased” to try to keep this from happening to anyone else.

“I plan to notify every congressman, senator, whoever, and let them know what’s going on, see if we can get this changed. I’m hoping I can be the beginning of the change,” Howard said.

Howard says once she started researching this problem, she was surprised by how many other people have been through it. She says when she went to her bank to reopen her account, they told her she was the second customer that day who had been wrongly declared dead.
